Documentation Requirements
This application will require:
- Proof of Residency – Proof of residency or legal residency for every member in the household.
- Proof of Income – Must submit proof of current income for the past 30 days that fall within the application date, for all household members 18 years and older living in the home.
- Award Letters – Current year social security, supplemental security Income, disability, unemployment letter, or a bank statement within the last 30 days of intake application showing an income deposit.
- Notarized Letter from Employer
- W-2 or 1099 Income Tax Statements
- Active CPS Account – Documentation needed includes a copy of the CPS energy bill and account number.
- Photo ID(s) for Account Holder – Texas ID/driver’s license/matricular consular or passport.
- Social Security Number of Account Holder – Last four digits only.
