Employees and former employees have the right to receive copies of their 1095-C Forms for the 2025 tax year.
To request a copy of your 1095-C, please send an email to [email protected] with the subject line titled “2025 1095-C Request” and provide your name and contact information so that we can contact you to verify and arrange a method for you to receive a copy of your requested form.
You can also mail-in a request for a copy by sending in your name and contact information to:
